Coordination of Patient Services

Every home health agency is required to ensure that all services effectively compliment each other and support the objectives outlined in the plan of treatment. The clinical record and minutes of Interdisciplinary Team conferences should demonstrate that all personnel providing services to any one patient communicate with one another and jointly evaluate and plan patient services. A written progress report about each patient must be sent to the physician at least every 60 days.
